Field Manual in Emergency Medicine for



  1. Adenosine: 6 mg. Repeat in 1-2 min 12mg and additional 12mg prn
  2. Atropine: 0.5-1.0 mg IV/EET
  3. Bretyllium: 5 mg/kg IV, f/u with 10 mg/kg if needed
  4. Dexamethasone (Decadron): 10mg IV followed by 4mg IV q64 for spinal cord compression
  5. Dopamine: 400 mg in 250 ml D5W@ 2-20mcg/mg/min
                         70 kg = 13 ml/h=5mcg/kg/min.
                         2-5 mcg/kg/min = dopaminergic,
                         5-10 mcg/kg/min = beta effect
  6. Epinephrine: 1mg (1:10,000) IV/EET for cardiac arrest.
                             0.3-0.5 ml of 1:1000 solution SQ for anaphylaxis
  7. Hydrocortisone (Solu-Cortef): 100mg IV STAT for adrenal crisis and for "stress dose steroids".  May be followed as needed by 100-200mg q6h x 24 hrs.
  8. Lidocaine:  1mg/kg IV followed by 0.5 mg/kg q10min as needed to max of 3mg/kg
                          Maintenance 2g in 250ml D5W @1-4 mg/min
  9. MgSO4: Eclampsia:-1-4 g IV over 2 min, drip 5g in 250ml D5W @ 1g/h(50ml/h)
  10. Mannitol: 12 g/kg in 30-60 min for CNS herniation
  11. Methylprednisolone (SoluMedrol): 125mg IV for asthma. If spinal trauma-30mg/kg IV in 15min followed by 5.3 mg/kg/hr x 23 hrs.
  12. Naloxone (Narcan): 0.01 mg/kg or 0.4-2mg IV/IM/SQ/ETT
  13. Nitroglycerin (Tridil): 50mg in 250 ml D5W@ 5mcg/min (1.5ml/hr). Titrate to pain and BP
  14. Nitroprusside (Nipride):50mg in 250ml D5W@ 0.3mcg/kg/min (6ml/h in 70 kg man)
  15. Norepinephrine (Levophed): 4mg in 500ml D5W @2-4mcg/min (20ml/h = 3mcg/min)
  16. Phenytoin (Dilantin): 1g in NS no faster than 50mg/min
  17. Procainamide (Pronestyl): 20mg/min IV until:
    1) QRS or PR>50%
    2) Arrhythmia controlled
    3) < BP
    4) total of 17mg/kg or 1000 mg given
  18. Thiamine : 100 mg IM
